What You'll Need to Apply:
To apply, you'll need to complete our APPLICATION. You'll also may need to supply these documents:

Please note: If you are asking us to help you pay a bill, the bill must be in your name.
For Covid Relief Funding you will need to provide an official tax document, a copy of the lease or bill to be paid, proof that the loss of income is related to Covid-19 like a furlough letter or unemployment statement, proof of income for 2 weeks and bank statements for the last month. See Check list on the next page for more info.
* A photo ID for each adult living at
the applicant's address
* A Social Security card, ITIN card or application for each person (adult
and child) living at the address
* Proof of how the household is
paying its bills
We need three months of paychecks, an unemployment award letter, a Social Security award letter, and three months bank statements.
